Anna deduces that McBain and the other escapees are at PCU.  There, McBain, Lucy and Rafe prepare for final battle with Caleb on Spoon Island.  

Meanwhile, Sam and Danny are Caleb's prisoners in Wyndemere.  Anna discovers that the escapees are armed and fears blood will soon be spilled.  Kate makes the horrifying realization that Connie caused Trey's death.  Kate, reeling, lashes out at Sonny for taking to Connie's bed after all the things she'd done.  

Michael comforts Starr, who mourns Cole and Hope on this, the first anniversary of their deaths.  Despite all the loss and heartache they've endured, they're glad to have found each other.  Later, Michael escorts Starr off, unaware that a mysterious, unrevealed figure has been watching all along. 

Patrick attends a support meeting for his drug problem, where he reflects on the anniversary of Robin's death.  Elsewhere, Sabrina finds out that her future in medicine remains in limbo.  Patrick happens on Sabrina lighting a candle for the deceased police officer in the hospital chapel.  Patrick learns a little about Sabrina's childhood and the events the brought her to Port Charles.  A run-in with Sabrina has Steve confessing to Elizabeth his worry that Olivia's vision about his stabbing is due to come true.  Later, we learn he might have reason to worry when Olivia receives a visitor in the form of Heather.
